battle cry dom essay Byline: By HUGH BROGAN; Hugh Brogan is the author of ''The Longman History of the essay, United States of America.'' Lead: LEAD: BATTLE CRY OF FREEDOM The Civil War Era. By James M. McPherson . Illustrated. 904 pp. New York: Oxford University Press. $30; after July 1, $35. BATTLE CRY OF FREEDOM The Civil War Era. By James M. Admission! McPherson. Illustrated. 904 pp. New York: Oxford University Press. $30; after July 1, $35. No doubt someone in the Library of Congress knows exactly how many books have been published on the Civil War, but everyone knows, without counting, why a new one is to be greeted somewhat warily. The Civil War is the most worked-over topic in United States history, one of the most written about in the history of the world.

It is therefore a particular pleasure to report that ''Battle Cry of Freedom'' easily overwhelms all such doubts. It is the best one-volume treatment of its subject I have ever come across. It may actually be the best ever published. It is comprehensive yet succinct, scholarly without being pedantic, eloquent but unrhetorical. It is gcse compellingly readable. I was swept away, feeling as if I had never heard the case studies, saga before. It is most welcome.

I am conscientiously bound to report that it is not quite perfect. The maps are accurate and intelligent, but they are printed on a gray ground that makes the gray lines and essay gcse, arrows of the Southern forces hard to see. ''These defenders of the South doth protest too much'' is a horrible solecism. And there never was such a place as the ''Kingdom of Palermo.'' I can think of nothing else to complain about. It is easier work to summarize James M. McPherson's many merits as a historian. Of these the greatest is phd thesis surely his literary skill. It shows in essay, little things and great. For instance, he knows how to use precise details to bring the past, even the overfamiliar past, to vivid life again.

I was aware that the Army of the Potomac suffered shocking ill health (largely from bad sanitation) until Gen. George McClellan took it in hand, but I had never heard that in 1861, in the western Virginia theater of war, the Confederates suffered as badly because their farm-boy recruits went down with measles and mumps, to which they had never before been exposed. Unfairly (these children's complaints are killers for adults), ''measles and mumps'' has a slightly ludicrous ring; precisely because of phd thesis on ofdm, that I shall never again forget how Robert E. Lee was dealt his first defeat, and how West Virginia got its statehood. Mr. McPherson is wonderfully lucid. Again and again, hopelessly knotty subjects (for example, Lincoln's relations with the radical Republicans) are painlessly made clear. Above all, everything is in a living relationship with everything else. This is magic. Accounts of the Civil War usually sacrifice either detail (often important detail) to narrative flow, or narrative to detail.

Mr. Essay Gcse! McPherson does neither. Omitting nothing important, whether military, political or economic, he yet manages to ethics, make everything he touches (say, Union finances or the essay gcse, prisoner-of-war camps) drive his narrative forward. And though no one could write less like those ostentatious Civil War buffs (Bruce Catton springs to mind) whose prose is unrelentingly oratorical, at times he adds to the reader's pleasure either by studies quiet touches of humor or by gcse literary contrivances so bold as to force themselves on our attention. The supreme example is his omission of Lincoln's assassination. One moment the President is making a speech, with John Wilkes Booth snarling in the crowd (''Now, by essays against God, I'll put him through. That is the last speech he will ever make''). In the next sentence a new chapter begins; Good Friday, 1865, is already in the past and Mr. McPherson is pressing on to describe the very last incidents of the essay gcse, war.

It is breathtaking, almost insolent; but it is right. Everybody knows the story of the murder. Lincoln's sudden disappearance from the book gives us something of the shock that his contemporaries felt when he died - and it experience resume, the author has no space at essay gcse his disposal for redundant details. The book is opened with equal, though less obtrusive, skill. This is admission not only an account of the Civil War, it is a volume in the Oxford History of the United States, and so it begins with a masterly description of the republic at midcentury - a divided society, certainly, and a violent one, but not one in which so appalling a phenomenon as civil war is likely. So it must have seemed to most Americans at essay the time. Slowly, slowly the remote possibility became horrible actuality; and Mr. McPherson sees to it that it steals up on his readers in the same way. This is historical writing of the highest order, conveying perhaps the most important lesson of all: that we are not always masters of our fate, even when we most need to be. Essays! It is for gcse, this and similar reasons that the ethics case studies, Civil War must still be accounted a great and essay gcse, necessary theme, mattering more to citizens of the United States than any other in their past. Essays Abortion! In the right hands, it teaches inescapably the essay gcse, dreadful truth that it is possible for no more than ordinarily sinful men and women, and for politicians of conspicuous ability and patriotism, and for a country enjoying unrivaled prosperity and freedom, to make deadly and irretrievable blunders.

Americans did it to themselves. Nobody else can be blamed. Unless every generation receives the lessons of the Civil War in all humility they may commit such blunders again (as indeed the war in naturalism essay, Vietnam showed). Essay! But the story has often been perverted to teach other lessons (the wickedness of studies, capitalists or damyankees, the righteousness of the gcse, Southern cause) or to emile, teach no lessons at all, to gcse, be an entertainment for vocabulary, drum-and-trumpet hobbyists. Worst of all, it has been pridefully asserted that, since it belongs to a world now remote, it is gcse unimportant and irrelevant. It is a very good thing that ''Battle Cry of essays abortion against, Freedom'' is now available to refute all such heresies. I do not think that this epic of doom, this tale of a disaster perhaps implicit in the failure of the Founding Fathers to put slavery into the way of extinction, can be fully conveyed except in narrative form. Mr. McPherson argues that narration is the essay gcse, only mode by which what he calls the contingent factor in the Civil War can be made clear.

For he does not think Northern victory was inevitable. The Confederacy had the it experience, simpler war aim - to survive until the essay gcse, Union gave up in despair. The Northern will to college admission vocabulary, go on fighting was thus the essential precondition of victory, and if Antietam, or the Gettysburg campaign, or the election of 1864 had gone the other way, as they easily might have done, that will might have cracked. Gcse! Only after the fall of Atlanta and Sheridan's defeat of Jubal Early had insured Republican victory in 1864 did the defeat of the South become certain, or so Mr. McPherson thinks; and by his narrative he argues his readers into thinking so too. Admission Vocabulary! Events, he reminds us, are frequently unpredictable, and frequently they master men. The lesson is not new, but he handles it freshly, and so rebukes a generation of historians infatuated with theory, structure, numbers and essay, the mere accumulation of data. On only one matter of importance did I feel inclined to take issue with Mr.

McPherson. His seventh chapter bears the title, ''The Revolution of 1860,'' by which he refers to college essay online, the election of Abraham Lincoln. Chapter Eight, which deals with secession, is called ''The Counterrevolution of 1861.'' Here, I think, he lets his vocabulary run away with him. ''Revolution'' is a much abused word. In politics it must surely mean something more than a victory in a regular election. No one can deny that the Republicans stood for something new in American history, but the forces they represented - liberalism, urbanism, industrial capitalism, nationalism - had been steadily gaining ground for decades, and some of the groups that gave them victory at the polls, especially the Western farmers, had been around for essay, longer than the Republic.

True, these forces had been held in check for a few years by Southern obstruction, but in against, 1860 all they actually did in revenge was to gcse, elect Lincoln, and they were much assisted by resume the collapse of the Democratic Party. They were not given time to essay gcse, attempt more, for their vic-tory was immediately followed by the real revolution of 1860 -the planters' revolution - the ethics case studies business, secession of South Carolina and her sister states, which were as committed to breaking up a long-established Government as the Bolsheviks were in 1917, and more so than the French revolutionaries of 1789. They could only be defeated by war and the emancipation revolution (or counterrevolution); as Lincoln said, ''We must free the essay gcse, slaves or be ourselves subdued.'' The work of dismantling racial segregation in the North was begun; the national Government was greatly strengthened to meet the needs of war; and the complete Republican economic program was passed into law. It Experience Resume! It was indeed the second American Revolution; but it would not have happened nearly so fast, even under so astute and patient a President as Lincoln, if the South had remained in the Union. In short, to my mind the election of 1860 was only a step on essay, the way to revolution (just as the election of 1932 was), not a revolution itself. To be sure, Jefferson Davis said it was an college admission vocabulary abuse of language to call secession a revolution. He and his followers had left the Union ''to save ourselves from gcse a revolution,'' an resume assault on property in slaves.

But Davis was not a very wise man or an accurate reasoner. He had helped launch an onslaught on the old American political structure, which had sheltered slavery so comfortably; it is hardly surprising that others - abolitionists, Republicans, blacks - for their own purposes enlarged the essay, gap thus opened. As Davis contemplated the ruined postwar South, he should have acknowledged his responsibility. But I don't think he ever did. Disagreements on points of emphasis do not deserve much emphasis themselves. ''Battle Cry of naturalism essay, Freedom'' is in every respect a deeply satisfying book. Its illustrations are particularly good. The scholarship is as up-to-date and gcse, complete as anyone could reasonably wish. Resume! If at various places it is surprising not to find reference to a particular scholar or treatise, it is on the whole much more surprising to see how much reading and reflection Mr. McPherson has been able to cram into essay a life of no more, presumably, than the usual number of hours per day. The notes at without the bottom of the page (hooray!) will help explain to scholars why Mr. McPherson is essay gcse so often able to make fresh sense of on ofdm, old puzzles.

The bibliographical note at the end will provide the novice with an excellent guide to further reading. And at essay gcse only $30 until July, the book is even cheap, or at least wonderful value for the money. LINCOLN: THE RIGHT IDEAS, THE RIGHT GENERALS. It Experience Resume! James M. McPherson, the author of ''Battle Cry of Freedom,'' has a different explanation of the outcome of the Civil War. The North's victory, he said, ''is often attributed to its superior resources. But the greatest population doesn't always win - after all, Vietnam won. In certain campaigns, the South was outgeneraled. For example, at Gettysburg it wasn't only Southern mistakes that resulted in the Confederate defeat but Union leadership.'' In a telephone interview, he said that the President as Commander in essay, Chief as well as political leader played a strong role during the ethics studies business, war. ''Lincoln held the essay gcse, North together, picked the right generals and, finally, moved in world without, the direction of gcse, defeating slavery under his war powers,'' Mr.

McPherson said. All along, he believed slavery was morally wrong. Union victory insured that the Northern vision would become the American vision.'' Mr. McPherson, who is a professor of phd thesis on ofdm, history at essay Princeton University, has a family connection to the Civil War, although none that he knows of to James B. McPherson, a Union general who was killed when he inadvertently rode behind Confederate lines and then refused to surrender. The ancestors he knows about were somewhat less exalted. ''One remained a private and the other took an examination when he was 19 and became a lieutenant in the 22d U.S.

Colored Troops. He ended up a captain. That's how a lot of young soldiers were commissioned - as white officers of black regiments.'' ''Battle Cry of naturalism, Freedom,'' which is part of the Oxford History of the gcse, United States being written under the college essay online, general editorship of gcse, C. Vann Woodward, covers the era from the Mexican-American War to college essay vocabulary, Appomattox, but it is the essay gcse, slavery issue that compelled Mr. McPherson's interest. His first book, published 22 years ago, was about the abolitionists and blacks.

He is now in San Marino, Calif., doing research at without the Huntington Library for his next book, titled ''Abraham Lincoln and the Second American Revolution.'' HERBERT MITGANG.

Case Study: A Patient With Type 2 Diabetes Working With an Advanced Practice Pharmacist to Address Interacting Comorbidities. Advanced practice pharmacists in the field of diabetes work collaboratively with patients’ medical providers, often in primary care settings or in close proximity to the providers’ practices. They help to integrate the essay gcse, pharmaceutical, medical, education/ counseling, and direct patient care activities necessary to meet patients’ individual self-management and diabetes care needs. Patient education and self-management behavioral change are underpinnings of pharmaceutical care, and not only as they directly relate to against the use of medications. Pharmacists, especially those who are certified diabetes educators (CDEs), frequently provide diabetes patients with education not only on medications, but also on the overall disease state, nutrition, physical activity, decision-making skills, psychosocial adaptation, complication prevention, goal setting, barrier resolution, and cost issues.

In addition to these substantial education responsibilities, advanced practice pharmacists who are Board Certified–Advanced Diabetes Managers (BC-ADMs) play an expanded role that encompasses disease state management. This includes performing clinical assessments and limited physical examinations; recognizing the need for additional care; making referrals as needed; ordering and interpreting specific laboratory tests; integrating their pharmacy patient care plans into patients’ total medical care plans; and entering notes on patient charts or carrying out other forms of written communication with patients’ medical care providers. Depending on state regulations and physician-based protocols, some advanced practice pharmacists can prescribe and adjust medications independently or after consultation with prescribing clinicians. The clinical activities of BC-ADM pharmacists are not carried out independent of referring, collaborative practitioners. Rather, they are complementary to and serve to enhance the diagnostic, complex physical assessment, and gcse management skills of medical providers. The following case study illustrates the pharmacotherapeutic challenges of diabetes with other comorbidities, which can lead to potential drug-drug and drug-disease interactions. Although it does not offer detailed solutions to such problems, this case does describe the process of patient care and problem resolution as approached by advanced practice pharmacists. B.L. is a 58-year-old white woman who has been referred to the pharmacist clinician for pharmacotherapy assessment and diabetes management. Her multiple medical conditions include type 2 diabetes diagnosed in emile zola naturalism, 1995, hypertension, hyperlipidemia, asthma, coronary artery disease, persistent peripheral edema, and essay gcse longstanding musculoskeletal pain secondary to a motor vehicle accident. Ethics Case Business! Her medical history includes atrial fibrillation with cardioversion, anemia, knee replacement, and multiple emergency room (ER) admissions for asthma.

B.L.’s diabetes is currently being treated with a premixed preparation of 75% insulin lispro protamine suspension with 25% insulin lispro preparation (Humalog 75/25), 33 units before breakfast and 23 units before supper. Essay Gcse! She says she occasionally “takes a little more” insulin when she notes high blood glucose readings, but she has not been instructed on the use of an insulin adjustment algorithm. Her other routine medications include the fluticasone metered dose inhaler (Flovent MDI), two puffs twice a day; salmeterol MDI (Serevent MDI), two puffs twice a day; naproxen (Naprosyn), 375 mg twice a day; enteric-coated aspirin, 325 mg daily; rosiglitazone (Avandia), 4 mg daily; furosemide (Lasix), 80 mg every morning; diltiazem (Cardizem CD), 180 mg daily (per cardiologist consult); lanoxin (Digoxin), 0.25 mg daily (per cardiologist consult); potassium chloride, 20 mEq daily; and fluvastatin (Lescol), 20 mg at bedtime. Medications she has been prescribed to take “as needed” include sublingual nitroglycerin for it experience resume chest pain (has not been needed in the past month); furosemide, additional 40 mg later in essay gcse, the day if needed for swelling (on most days the additional dose is needed); and it experience albuterol MDI (Proventil, Ventolin), two to four puffs every 4–6 hours for shortness of breath. She denies use of nicotine, alcohol, or recreational drugs; has no known drug allergies; and is up to date on her immunizations. B.L.’s chief complaint now is increasing exacerbations of gcse, asthma and the need for phd thesis on ofdm prednisone tapers. She reports that during her last round of prednisone therapy, her blood glucose readings increased to the range of 300–400 mg/dl despite large decreases in gcse, her carbohydrate intake. She reports that she increases the frequency of admission essay, her fluticasone MDI, salmeterol MDI, and albuterol MDI to four to five times/day when she has a flare-up. However, her husband has been out of work for more than a year, and their only source of income is essay, her Social Security check. Against! Therefore, she has been unable to purchase the fluticasone or salmeterol and so has only been taking prednisone and essay albuterol for recent acute asthma exacerbations. B.L. reports eating three meals a day with a snack between supper and bedtime.

Her largest meal is college admission online, supper. Essay Gcse! She states that she counts her carbohydrate servings at each meal and essay online is “watching what she eats.” She has not been able to exercise routinely for gcse several weeks because of bad weather and her asthma. The memory printout from her blood glucose meter for the past 30 days shows a total of 53 tests with a mean blood glucose of 241 mg/dl (SD 74). Phd Thesis On Ofdm! With a premeal glucose target set at 70–140 mg/dl, there were no readings below target, 8% within target, and 91% above target. Essay! By comparison, her results from the same month 1 year ago averaged 112 mg/dl, with a high of world without art essay, 146 mg/dl and essay gcse a low of phd thesis, 78 mg/dl. B.L. is well-appearing but obese and is in no acute distress. A limited physical exam reveals: Weight: 302 lb; height 5?1?

Blood pressure: 130/78 mmHg using a large adult cuff. Pulse 88 bpm; respirations 22 per minute. Lungs: clear to auscultation bilaterally without wheezing, rales, or rhonchi. Lower extremities +1 pitting edema bilaterally; pulses good. B.L. reports that on the days her feet swell the gcse, most, she is active and in an upright position throughout the day.

Swelling worsens throughout the day, but by the next morning they are “skinny again.” She states that she makes the decision to take an extra furosemide tablet if her swelling is excessive and ethics case studies painful around lunch time; taking the essay gcse, diuretic later in art essay, the day prevents her from sleeping because of nocturnal urination. For the gcse, sake of college admission essay, brevity, only abnormal or relevant labs within the past year are listed below. Hemoglobin A 1c (A1C) measured 6 months ago: 7.0% (normal range: 5.9%; target: 7%) Creatinine: 0.7 mg/dl (normal range: 0.7–1.4 mg/dl) Blood urea nitrogen: 16 mg/dl (normal range: 7–21 mg/dl) Sodium: 140 mEq/l (normal range: 135–145 mEq/l)

Potassium: 3.4 mg/dl (normal range: 3.5–5.3 mg/dl) Calcium: 8.2 mg/dl (normal range: 8.3–10.2 mg/dl) • Total cholesterol: 211 mg/dl (normal range 200 mg/dl) • HDL cholesterol: 52 mg/dl (normal range: 35–86 mg/dl; target: 55 mg/dl, female) • LDL cholesterol (calculated): 128 mg/dl (normal range: 130 mg/dl; target: 100 mg/dl) Initial LDL was 164 mg/dl. • Triglycerides: 154 mg/dl (normal range: 150 mg/dl; target: 150 mg/dl) Liver function panel: within normal limits. Urinary albumin: 30 ?g/mg (normal range: 30 ?g/mg) Poorly controlled, severe, persistent asthma.

Diabetes; control recently worsened by asthma exacerbations and essay treatment. Dyslipidemia, elevated LDL cholesterol despite statin therapy. Persistent lower-extremity edema despite diuretic therapy. Hypokalemia, most likely drug-induced. Hypertension JNC-VI Risk Group C, blood pressure within target and stable. Coronary artery disease, stable. Chronic pain secondary to previous injury, stable. Status post–atrial fibrillation with cardioversion. Status post–knee replacement. Financial constraints affecting medication behaviors. Insufficient patient education regarding purposes and role of specific medications.

Wellness, preventive, and routine monitoring issues: calcium/vitamin D supplement, magnesium supplement, depression screening, osteoporosis screening, dosage for daily aspirin. Strand et al.1 proposed a systematic method for college admission vocabulary evaluation of and intervention for a patient’s pharmacotherapy, using a process called the Pharmacist’s Work-Up of Drug Therapy (PWDT). Essay Gcse! The PWDT has been modified by subsequent authors,2–4 but the process remains grounded in the following five questions: What are reasonable outcomes for this patient? Based on current guidelines and literature, pharmacology, and pathophysiology, what therapeutic endpoints would be needed to achieve these outcomes? Are there potential medication-related problems that prevent these endpoints from being achieved? What patient self-care behaviors and medication changes are needed to address the medication-related problems? What patient education interventions are needed to college enhance achievement of these changes?

What monitoring parameters are needed to essay gcse verify achievement of goals and detect side effects and toxicity, and how often should these parameters be monitored? Clinical outcomes are distinctly different from therapeutic or interventional endpoints. The former refers to the impact of treatment on patients’ overall medical status and quality of life and should emphasize patient-oriented evidence that matters (POEMs) rather than disease-oriented evidence (DOEs). Therapeutic endpoints include the anticipated and desired clinical effects from case drug therapy that are expected, ultimately, to achieve the desired outcome(s). As such, therapeutic endpoints are used as surrogate markers for achievement of outcomes. Essay Gcse! Commonly, more than one endpoint will be needed to achieve an outcome.

For example, near-normal glycemic control and it experience resume normalization of blood pressure (endpoints) would be necessary to significantly reduce the risk of end-stage renal disease (outcome). Therapeutic endpoints should be specific, measurable, and achievable within a short period of time. Achievement of clinical outcomes usually cannot be determined except by long-term observation or retrospective analysis. Outcomes and endpoints for any given patient should be determined collaboratively between patient and provider before selecting or initiating pharmacotherapy or nonpharmacological interventions. Essay! Taking the time to zola naturalism essay identify these components up front (and periodically revise them later on) helps ensure that subsequent medications or strategies are appropriately directed. It further ensures a common vision and commitment for ongoing patient care and self-management among the care team (including the patient), thus maximizing the potential for optimal disease control and patient satisfaction. The outcomes and endpoints for a patient such as B.L. are numerous and obviously would not be addressed or attained in a single session. Therefore, after desired outcomes and endpoints are determined, they should be prioritized according to medical urgency and gcse patient preference. Implementation and goal setting related to against these priorities can then be undertaken, thus establishing a treatment plan for the eventual attainment of the full list.

During ongoing and follow-up visits, this care plan should be reviewed and modified as indicated by changes in patient status, preferences, and medical findings. Examples of desired outcomes and endpoints for B.L. are given in Tables 1 and 2. Essay! For the on ofdm, sake of brevity, these tables are not intended to essay gcse be inclusive. Medication-Related Problems and Proposed Interventions. With agreement between patient and clinician concerning desired outcomes and endpoints, the next logical step is to evaluate whether the current treatment plan is likely to achieve those goals, or, if treatment is to on ofdm be initiated, which therapies or interventions should be selected. According to Strand et al.,1 a medication-related problem is essay gcse, any aspect of a patient’s drug therapy that is interfering with a desired, positive patient (therapeutic) outcome or endpoint.

The PWDT proposes a systematic and without art essay comprehensive method to essay identify, resolve, or prevent medication-related problems based on the following major categories: No indication for a current drug. Indication for world art essay a drug (or device or intervention) but none prescribed. Wrong drug regimen (or device or intervention) prescribed/more efficacious choice possible. Too much of the correct drug. Too little of the correct drug. Adverse drug reaction/drug allergy. Drug-drug, drug-disease, drug-food interactions. Patient not receiving a prescribed drug. Routine monitoring (labs, screenings, exams) missing.

Other problems, such as potential for essay overlap of adverse effects. Once problems are identified, resolutions must be developed, prioritized, and implemented. Patient or caregiver input is ethics studies, especially helpful at this stage because the individual can describe subjective as well as objective data, expectations or concerns that may be affecting drug therapy, and gcse deficits in drug knowledge, understanding, or administration. Resolutions may result from case studies business numerous strategies, including dose alteration, addition or discontinuation of medication, adjunct medications, regimen adjustment, complementary therapies, instruction on medication administration or devices, disease or medication education, development of “cues” as compliance reminders (e.g., pill boxes), and essay identification of essay vocabulary, ways to avoid, detect, or manage side effects or toxicities. Needless to say, the involvement of essay, patients and family or caregivers is art essay, critical for gcse successful implementation of most resolution strategies and for phd thesis optimal disease management. Because of the essay, extent of without, B.L.’s medication-related problems and potential interventions (Tables 3 and gcse 4), it was agreed to tackle first her asthma exacerbations and emile zola naturalism essay high blood glucose levels. Essay! To this end, B.L. was counseled about the role of maintenance asthma medications versus rescue drugs. The root of her confusion between these agents was easy to understand—because the prednisone and fluticasone were both called steroids, it seemed likely that the tablets were a cheaper and easier way to take the medicine.

Likewise, since albuterol and phd thesis on ofdm salmeterol were both called bronchodilators, it seemed that the albuterol was the cheaper way to take the gcse, medicine. Having grasped the concept of asthma prevention, she was willing to convert to a product combining fluticasone and salmeterol (Advair Diskus) for maintenance/prevention and to reserve the albuterol for quick relief of acute symptoms. Free samples of the new product were dispensed, and phd thesis on ofdm B.L. was enrolled in the manufacturer’s indigent drug program for subsequent supplies. She was further instructed on the use of a peak flow meter and gcse advised to monitor her readings and symptoms. At the next visit, these data will be used to determine her maximal expiratory effort (“personal best”) and to construct an asthma action plan. B.L.’s insulin was changed to a basal-based regimen utilizing bedtime glargine (Lantus) insulin and premeal lispro (Humalog) insulin. On Ofdm! Education was provided on this dosing concept. She and essay the pharmacist discussed how this regimen can give greater flexibility in dosing, especially for responding to world without art essay changes in diet, exercise, and disease exacerbations or medications. She was also given an essay initial supplementary adjustment algorithm (sliding scale) to correct for any temporary elevation of emile naturalism, blood glucose.

She agreed to test four times daily and to record her blood glucose results, carbohydrate intake, and insulin doses. At the next visit, these data will be used to modify the adjustment algorithm and to construct a prospective algorithm for matching premeal bolus insulin to the anticipated carbohydrate intake (insulin-to-carbohydrate ratio). The final interventions for this visit were to increase the dose of potassium chloride and change the gcse, fluvastatin to atorvastatin (Lipitor) to case studies further reduce B.L.’s LDL cholesterol. Medication education for essay gcse atorvastatin was provided, and patient questions were answered. Other medication-related problems and interventions identified for B.L. are listed and without briefly discussed in gcse, Tables 3 and 4. For the sake of brevity, these lists are not inclusive nor are all pharmacotherapy issues discussed.

Monitoring for college admission essay vocabulary Effectiveness, Side Effects, and Toxicity. The last step in essay, the PWDT process is to develop a plan to evaluate the patient’s progress in attaining desired outcomes, therapeutic endpoints, and behavior changes; to assess effectiveness of pharmacotherapy; and to identify side effects, drug interactions, or toxicity issues that need to be addressed. The monitoring/follow-up component is the phd thesis on ofdm, most tedious aspect of the PWDT. For each medication or intervention, key parameters must be identified as markers for effectiveness, for side effects, for drug interactions, and for toxicity. In addition, the time frame and process for assessing those parameters must be determined. Finally, the gcse, desired range for the parameter must be listed or a “decision point” must be identified to signal that additional action will be required. It should be noted that only a limited number of world without, parameters are selected for a given patient. For example, it is not necessary to list and monitor for every possible side effect with equal intensity and frequency. Essay! Selection of the monitoring parameters is based on without art essay the positive effects (efficacy) that are most important to the care of that patient, as well as the gcse, adverse effects (side effects, toxicity, or drug interactions) that are most important to essays abortion against avoid for the safety of that patient or to which that patient is most prone. Because the monitoring component is usually extensive, examples listed for B.L. in Table 5 have been limited to gcse three of the essay online vocabulary, medication or regimen changes that were made at the first pharmacist visit: switching from fluvastatin to gcse atorvastatin; switching from two shots of premixed 75/25 lispro to bedtime glargine with premeal lispro; and substituting the combination inhaler product for her fluticasone and salmeterol MDI prescriptions.

Because atorvastatin and abortion fluvastatin differ chemically, the gcse, monitoring parameters for this change are similar to college admission essay online those for initiation of a new medication. Monitoring for the new insulin regimen (basal insulin with premeal bolus) focuses primarily on glycemic control patterns and hypoglycemic episodes. Essay Gcse! Because B.L. has previously used the two ingredients of her new inhaler product (fluticasone and salmeterol) without adverse effect, monitoring of studies business, her new asthma therapy is focused on effectiveness, tolerance of inhalation of its dry powder formula, and use of the administration device. Diabetes patients with multiple co-morbidities have concerns about all of their problems, not just the diabetes; therefore, BC-ADM pharmacists must comprehensively explore all the gcse, ramifications of ethics business, comorbidities as well as patients’ feelings, expectations, and concerns for essay total health. B.L. is a good example of abortion against, this; even though her referral was for “diabetes management,” her greatest concern at this visit was her asthma exacerbations. As can be seen in gcse, this case, each coexisting disease or coprescribed drug has a domino effect, affecting other diseases or drugs and ultimately affecting quality of life. With input from ethics case business B.L., the gcse, pharmacist clinician was able to college admission online vocabulary develop a PWDT that addresses her diabetes as well as her other health care needs.

B.L. was able to essay leave the health center with a few achievable self-care goals and medication changes that address her acute concerns and with the knowledge and confidence that, at each subsequent visit, additional progress will be made toward her personalized health status goals. Examples of B.L.’s Desired Outcomes. Examples of emile zola, B.L.’s Therapeutic Endpoints. Examples of B.L.’s Medication-Related Problems. Examples of B.L.’s Interventions (Prioritized and to be Implemented Accordingly)

Examples of essay gcse, B.L.’s Monitoring Plans.
